Application

For those who are interested in joining the Friends of Cook, here is the application. If you're unable to open the application, you may need to get the free Adobe Acrobat program first. There are also membership forms available at Cook Memorial Library or at the door of any of our events.

The prices are

• individual membership: $5
• lifetime membership: $50
